Our model focuses on improving the individual experience of care and improving the health of everyone in our communities – regardless of gender, age, income or ethnic and racial backgrounds – all while reducing costs and passing the savings on to our patients.

Creating opportunities for every person to achieve their highest level of health requires efforts both big and small. By focusing on prevention, early detection, healthy behaviors and access to care, we can continue the journey together to provide equitable care for all.

From improving breast cancer screening rates for Black and Hispanic women, to addressing maternal morbidity and organizing data to help identify disparities, we're proud to be a nationally recognized leader in addressing health equity.

A few of our recent successes include:

Breast cancer screenings

More than 48,000 breast cancer screenings were completed as of June 2023. Among those:
- We saw an increase of 6.3% among both Black and Hispanic women getting screened.
- A pilot program to increase the number of Spanish-speaking patients completing a screening mammography identified 310 Spanish-speaking women overdue for their screenings. Of the screenings completed, seven patients had a positive finding for breast cancer, and all were identified early.
- The program resulted in the prevention of an estimated 21 deaths from breast cancer.

Colon cancer screenings

More than 15,000 patients received colon cancer screenings in the last year. This is an 8.92% increase from the previous year, and 8.73% more for people of color.
The program prevented an estimated 345 colorectal cancer deaths.

Recovery support

More than 900 patients received recovery support for opioid use disorder.

We know that keeping you healthy is just as important as helping you when you're not. Today, health care organizations are increasingly rewarded for providing preventative and wellness care rather than treating you only when you're sick or injured.

It's a shift from a model rewarding volume of services over quality, and it reduces unnecessary services – which saves money. We pass those savings on to our patients for the highest quality care at an affordable price.

Medicare Accountable Care Organization (ACO)

An ACO is a group of doctors, hospitals and other providers working together to provide Medicare patients with the best care at the lowest cost. Our ACO is the region’s largest and one of the best in the nation.
